1. What is your organization’s mission?

CSCS Programs provides multiple programs to assist aging and disabled persons. Serving all of Carbon and Stillwater counties, CSCS Programs provides access and direct services that will assist our clients' independence to live a life with dignity and respect within our communities.

3. Provide a description of your organization including services offered, who your organization serves, service area, main events, etc.

CSCS Programs helps individuals, families, and caregivers with many Senior issues through the Information & Assistance services.  A comprehensive solution may involve several referrals addressing needs such as food assistance, utility assistance, Medicare/Medicaid helps, Social Security assistance, or other non-long-term care needs.

SHIP, State Health Insurance Program, offers one-on-one counseling and assistance to people and their families/caregivers with Medicare/Medicaid, or other insurance.

The Long Term Care (LTC) Ombudsman resolves complaints made for or by residents in care facilities. The Ombudsman educates consumers and LTC providers about residents’ rights and good care practices.  They also promote the development of resident and family councils.

The Commodity Supplemental Food Program (CSFP) provides a box of canned and dry foods, and cheese each month to qualified low-income recipients.

An Options Counseling session can be used to discuss short and long term care needs and to develop an action plan that works for you.

Care Chat Chums is a friendly phone visitor program for clients and volunteers.

CSCS Programs serves clients throughout Carbon and Stillwater counties.  We provide programs at 9 senior centers and 9 care facilities.  We distribute food boxes in both counties.  CSCS Programs office is located in Joliet, but we meet with clients and families at senior centers or care facilities, too.  We do utilize phone contacts to help with the rural distances.  We also provide programs to civic groups and communities as requested.

CSCS Programs hosts an annual summer picnic between the two counties.

Carbon/Stillwater Community & Senior Programs is a designated 501(c)3 nonprofit.

4. What will funds raised through Columbusmtgives.org be utilized for in your organization?

Funds raised through Columbusmtgives.org will be used for updating computers, software, and growing existing and new programs in Stillwater County.

5. Share any testimonials or great stories about your organization and how it enhances our community.

Senior lady contacted our office, CSCS Programs, needing assistance with signing up for a Medicare prescription drug plan.  Upon consultation with the client she had never signed up for a Medicare drug plan and would be subject to almost 12 years of penalties.  We signed her up for Social Security Extra Helps and her and her husband qualified for 50% subsidy.  This program paid the penalty and provided them with discounted monthly drug plan premiums and lower drug copays.  In addition, they qualified for a state Medicare prescription assistance program, BSRX, which helped to pay their monthly drug plan premium difference.  A services screen was done to see if there were any other needs.  We assisted them in completing Power of Attorney financial and healthcare forms as her daughter was coming to visit from another state and the client could get them signed when she came.  Our client was happy that CSCS Programs was able to help her and her husband in many ways.
